Persistent pain can reflect ongoing tissue or joint pathology, nerve injury or irritation, changes in pain processing, or a combination of factors. A careful evaluation helps define the dominant pain mechanism and identify treatment options that fit the diagnosis, function, and goals.

Chronic pain is not a single diagnosis.

Two patients can describe similar pain while the mechanisms driving their symptoms are very different.

Some persistent pain remains primarily related to a specific structure or ongoing mechanical problem. Other symptoms may be neuropathic, arising from injury or dysfunction involving the nervous system. In many patients, more than one mechanism contributes. Identifying those differences helps guide a more individualized treatment strategy.

Persistent pain can present in different ways.

The quality, distribution, triggers, neurologic symptoms, and effect on function all provide clues about what may be driving the pain.

Neuropathic pain

Burning, electric, shooting, tingling, or hypersensitive pain may suggest involvement of a peripheral nerve, nerve root, or another part of the nervous system.

Persistent pain

Pain may continue for months despite healing time or prior treatment and can become increasingly disruptive to activity, sleep, work, and quality of life.

Post-surgical pain

Persistent pain after surgery may reflect residual structural disease, nerve-related pain, scar-related symptoms, or another pain generator that requires careful reassessment.

Mixed pain mechanisms

Structural, inflammatory, neuropathic, and other pain mechanisms can overlap, making a diagnosis-driven evaluation especially important.

Define the pain mechanism before escalating treatment.

Evaluation begins by revisiting the diagnosis rather than assuming that persistent symptoms represent a single chronic pain condition. The history, examination, prior procedures or surgeries, imaging, neurologic findings, and response to previous treatments can all help clarify the dominant pain generator.

The goal is to identify what remains treatable, distinguish structural or nerve-related pain from competing causes, and understand how symptoms are affecting function before considering more advanced interventions.

Persistent pain deserves a strategy—not simply more procedures.

Treatment is most useful when it is tied to a clear clinical rationale and meaningful functional goals. The appropriate plan may combine rehabilitation, medication optimization, targeted interventions, and, for selected patients, neuromodulation or other advanced therapies.

Targeted interventions

When a specific pain generator can be identified, image-guided or minimally invasive procedures may be considered as part of a broader treatment plan.

Neuromodulation

For carefully selected patients with certain persistent neuropathic or chronic pain conditions, spinal cord stimulation or peripheral nerve stimulation may be considered after appropriate evaluation and prior treatment.

Common questions about neuropathic and persistent pain.

What does neuropathic pain feel like?

Neuropathic pain is often described as burning, electric, shooting, tingling, numb, or unusually sensitive. These features can suggest nerve-related pain, but the symptom description alone does not establish the diagnosis.

Why can pain continue after an injury or surgery has healed?

Persistent symptoms can have several causes, including residual structural problems, nerve injury or irritation, changes in pain processing, or a different pain generator than the one originally treated. A focused reassessment can help clarify what may still be contributing.

Does chronic pain mean nothing more can be done?

No. Persistent pain can often be approached in several ways. The most appropriate strategy depends on the diagnosis, dominant pain mechanism, prior treatment, functional limitations, and individual goals.

When is spinal cord stimulation considered?

Spinal cord stimulation may be considered for selected chronic pain conditions after careful evaluation of the diagnosis, prior treatments, medical and psychosocial factors, functional goals, and expected benefits and risks. A temporary trial is generally performed before permanent implantation.

How is peripheral nerve stimulation different from spinal cord stimulation?

Peripheral nerve stimulation targets a peripheral nerve associated with the painful region, while spinal cord stimulation delivers therapy through leads positioned in the epidural space near the spinal cord. The appropriate approach depends on the pain condition, anatomic target, prior treatment, and individual clinical picture.

Persistent pain does not have to mean repeating the same treatment.
