Lumbar radiculopathy occurs when a nerve root in the lower spine becomes irritated or compressed. Symptoms may include radiating leg pain, numbness, tingling, or weakness—often described more generally as sciatica.

Sciatica describes a symptom pattern—not a single diagnosis.

Radiating pain into the buttock or leg can occur when a lumbar or sacral nerve root is irritated, inflamed, or compressed.

Common causes include a disc herniation, degenerative narrowing around the nerve, and lumbar spinal stenosis. Determining which nerve is involved—and why—requires the symptom pattern to be connected with the examination and imaging when appropriate.

More than pain traveling down the leg.

The exact pattern depends on the nerve root involved and the underlying cause. Symptoms can extend from the low back or buttock into different regions of the thigh, leg, or foot.

Radiating leg pain

Sharp, burning, aching, or electric-like pain may travel from the buttock or lower back into the leg.

Numbness or tingling

Altered sensation may follow a recognizable distribution in the thigh, calf, ankle, or foot.

Weakness

Some nerve-root problems affect strength and may produce difficulty with specific movements of the leg, ankle, or foot.

Activity-related symptoms

Sitting, standing, walking, bending, coughing, or other activities may change symptoms depending on the underlying cause.

Find the reason the nerve is irritated.

Disc herniation can irritate or compress a nearby nerve root through mechanical pressure and inflammation. Foraminal or lateral recess narrowing can reduce the space available for a nerve as degenerative changes develop.

Lumbar spinal stenosis can affect one or multiple nerve roots and may produce radicular symptoms, neurogenic claudication, or both. Other conditions can mimic radiculopathy, so the diagnosis should not be made from leg pain alone.

Match the pain pattern to the nerve—and the cause.

Evaluation begins with the history and neurologic examination, including strength, sensation, reflexes, and provocative testing when appropriate. MRI is commonly used when imaging is indicated to evaluate discs, nerve roots, and areas of narrowing. In selected situations, electrodiagnostic testing may provide additional information.

Pattern mattersThe distribution of pain and sensory symptoms can help identify which nerve root may be involved.
Neurology mattersStrength, sensation, and reflex findings help determine whether there is objective nerve dysfunction.
Imaging mattersStructural findings are most useful when they correlate with the clinical presentation.

Treatment depends on the cause, severity, and neurologic findings.

Conservative care

Many patients improve with time and appropriate non-surgical treatment, which may include activity modification, therapeutic exercise or physical therapy, and medication strategies when appropriate.

Image-guided treatment

For selected patients, an epidural steroid injection or targeted nerve-root approach may be considered to reduce inflammation and improve symptoms while recovery progresses.

Surgical evaluation

Progressive neurologic deficit, severe persistent symptoms, or anatomy that requires decompression may warrant surgical evaluation. The timing depends on the clinical situation.

Common questions about lumbar radiculopathy and sciatica.

Are sciatica and lumbar radiculopathy the same thing?

Not exactly. Sciatica is a commonly used term for pain that radiates through the buttock and into the leg. Lumbar radiculopathy more specifically refers to symptoms or neurologic findings caused by irritation or dysfunction of a lumbar or sacral nerve root.

Does a herniated disc always require surgery?

No. Many patients with disc-related radicular symptoms improve without surgery. Treatment depends on symptom severity, neurologic findings, functional limitation, imaging, response to non-surgical care, and how the condition changes over time.

What is an epidural steroid injection intended to do?

An epidural steroid injection may be considered in selected patients to reduce inflammation around an irritated nerve root and improve radicular symptoms. It does not physically remove a disc herniation or reverse the structural narrowing that may be contributing to nerve irritation.

Why might my MRI show a disc problem on the opposite side or at another level?

Degenerative findings are common and are not always responsible for a patient’s symptoms. An imaging abnormality is most meaningful when its level and side correlate with the symptom distribution, neurologic examination, and overall clinical picture.

When should weakness or bowel and bladder symptoms be evaluated?

New or progressive weakness, significant sensory changes, numbness in the saddle region, or new bowel or bladder dysfunction warrant prompt medical evaluation. Certain neurologic presentations can require urgent assessment, particularly when symptoms are severe or rapidly changing.
